What Is a Geriatric Care Manager?


A Geriatric Care Manager (GCM) is a professional, who specializes in assisting older individuals and their families with long-term or short-term care arrangements. Care Managers have a minimum of a bachelor's degree but usually have a graduate degree in social work, psychology, nursing, counseling or gerontology.

The professional care manager's role is complex and varied. While the primary responsibility is the clients best interest and welfare, the care manager functions as an advocate, liaison and support system to his/her client. A gericare manager utilizes a comprehensive biopsychsocial assessment. Based on findings, a care plan will be designed to meet the individuals identified needs.

The care manager will maintain regular contact, open communication and awareness to the ever-evolving, constant changes and needs of the client.
A Geriatric Care Manager Can:

  • Conduct care-planning assessments to identify problems, eligibility for assistance, and need for services
  • Screen, arrange and monitor in-home help or other services
  • Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
  • Provide crisis intervention
  • Act as a liaison to families at a distance, making sure things are going well and alerting families to problems
  • Provide consumer education and advocacy
  • Offer counseling and support
