The Initial Step

The initial step is to conduct a careful assessment of an individual’s need for assistance. This comprehensive assessment is an evaluation of the individual’s physical, cognitive, emotional, financial and psychological condition. Based on this evaluation, Growing Options will develop a care plan to enable the elder to remain in their home as long as possible. With this plan we’ll arrange a variety of services and act as a liaison between the client and those contracted to provide the services. The goal is to preserve independence and maintain the safety and dignity of the client.

Following the assessment, Growing Options can provide:

Counseling Services – provide support to assist families and the elder adults. We offer client counseling, family or caregiver counseling and support groups focusing on:
  • Crisis intervention and prevention
  • Services Provided
  • Sorting through conflicts amongst family members
  • Providing guidance and options to assist with difficult decisions
  • Counseling to the elder parent for the acceptance of services

Healthcare Services – providing personalized healthcare services in the home for optimum well being to successfully stay in one’s own home environment. These services are tailored to individuals needs and can include:

  • Specialized Health Care – nursing care, physical therapy, occupational therapy, speech therapy
  • Home Health Aide – assistance with daily living skills such as dressing, bathing and personal care
  • Medication – arrange delivery of prescribed medication, allocation of daily quantities and monitoring proper usage
  • Out-patient Medical Services – provide transportation and accompaniment for the visit
  • Medical Equipment – rental or purchase of necessary home healthcare supplies
  • Companion Services – providing for emotional and social support
  • 24-hour emergency coverage – available 24 hours a day for all emergency healthcare issues

Homecare Services – provide homecare services in order to maintain a safe living environment which is functionally adapted to the increase needs of the senior:

  • HomeSafe Program – analysis of home structural changes to ensure a safe living environment. This includes coordination and monitoring of all specialty contractors used
  • Meal Planning – options offered include delivery of groceries, fully prepared hot meals, in home cooking by homemaker or arranging hot meals at community senior centers
  • Home Maintenance – coordination of home repairs, housekeeping and lawn care
  • Social Recreation – arranging and escorting to senior social centers
  • Transportation and Shopping for personal needs

Financial & Legal Assistance – to provide professional guidance and management specializing in senior legal and financial needs:

  • Personal Finances – assistance in, or full management of personal banking, bill paying and budgeting
  • Insurance – entitlement assistance in determining eligibility for insurance such as Medicaid, Medicare, VA benefits etc
  • Financial Planning – analysis of current income and assets to determine options available as needs increase
  • Legal Assistance – arrangement of a Power of Attorney for health and/or financial decisions

Monitoring Service – after the care plan is implemented, an on-going “monitoring” process is critical to ensure that the services provided continue to meet the needs of the client with the highest quality of care. Monitoring serves as an opportunity to:

  • Assess the elders satisfaction with the services
  • Review the medical, emotional and physical well being of the client
  • Evaluate the impact of the providers services
  • Modify the care plan and services to accommodate changing needs of the client
  • Provide documented feedback to the family of the elder
